U.S. health insurer Anthem Inc. says data breach likely began as early as Dec. 10. The insurer also warned members that the data breach is being used as a lure by online and telephone scammers. Anthem has promised to send letters – via the US Postal Service – to everyone affected by the breach by Feb. 20.
